Privacy Agreement

By completing this application form and/or submitting my curriculum vitae, I hereby consent to Husky Injection Molding Systems Ltd. and its affiliates (collectively "Husky") collecting, using and disclosing my information for the purposes of determining my suitability for the position for which I have applied or for any other position that Husky may reasonably determine I may be qualified.

I understand that:

• Husky may use my personal information to conduct a background check where required or permitted by law and contact my references whom I permit Husky to contact; and
• Husky may disclose my personal information to vendors hired to provide employment verification services for Husky.

I further understand that:

• my personal information is being collected in accordance with the laws of the jurisdiction that are applicable to the position for which I have applied;
• my personal information may be stored on a server or on premises located outside the jurisdiction in which it was collected;
• my personal information may be available in limited circumstances to United States authorities under United States law;
• if I am hired by Husky, my personal information may be used for purposes reasonably related to the maintenance of my employment relationship with Husky;
• if I am not hired by Husky, my personal information will be retained by Husky for a reasonable period of time for the purposes of statistical analysis, to improve the efficiency of Husky’s job application process and to facilitate any future job applications; and
• Husky’s privacy statement is available for my review at www.husky.ca, and if I have questions or concerns regarding Husky’s privacy practices, I may contact Husky’s Chief Privacy Officer.
